PURPOSE
To maintain the highest standard in deliver of personal care services and supports in Community Based Home like setting.
STANDARD
Berry's Adult Foster Care Home shall adopt a person-centered approach to disseminate evidence-based best practices principles to improve positive outcomes reducing risk within our power ensuring the quality of services to the individuals served we shall utilizing SWOT analysis system.
Definition of SWOT
SWOT is a method for strategic planning that analyzes objectives based on strengths, weaknesses, opportunities and threats (thus the acronym SWOT). SWOT accounts for both internal and external variables, and it may be used to evaluate any system, including an adult foster care facility.
S = Strengths
Strengths include all internal attributes of the company (system or organization) that may positively affect the identified objective. Depending on the objective, any given internal attribute may be either positive or negative. Factors may include marketing mix, personnel, personal care service capabilities and management. Any internal variable that could impact strategy will be included.
W = Weaknesses
Weaknesses include all internal attributes of the company that may adversely affect the identified objective. Depending on the objective, any particular internal attribute may be either positive or negative. These factors also may include marketing mix, personnel, personal care service capabilities and management. Any internal variable that may impact strategy will be included.
O = Opportunities
Opportunities include all external conditions that may positively affect the identified objective. Depending on the objective, any given external factor may be either positive or negative. Factors may include industry economics, technology, legislation and social conditions. Include any internal variable that could impact strategy.
T = Threats
Threats include all external conditions that may adversely affect the identified objective. Any particular external factor may be either positive or negative, depending on the objective. Industry economics, technology, legislation and social conditions are all possible factors. Any external variable that may impact strategy must be included.
QUALITY ASSURANCE SERVICE STANDARDS

To facilitate and encourage the importance of family/guardian involvement in the personal care and community living supports of the consumer in this foster care setting

To provide the highest degree of quality care in the least restrictive way so the needs and personal goals of the consumer can be met within the scope of our care based on the best practice principles in this industry

To ensure and maintain the consumers right to respect, privacy, dignity and confidentiality in all aspects of the consumers life in this foster care setting

To ensure the consumer has the opportunity to develop and or maintain living skills to successfully function and participate in group/community activities based on the acuity level of the consumer

To be free to raise and have resolved any complaints/disputes the consumer may have regarding the personal care and community support services received

To provide person-centered care we will provide sound management practices to ensure the best possible outcome for the consumer's mental status and physical well-being status within the scope of our care based on the best practice principles in this industry

To be free at all times from sexual, physical, and emotional abuse we will ensure that the human rights of each consumer placed in our care will be upheld within this facility based on the best practice principles in this industry

To provide 3 nutritious meals as scheduled and a daily snack based on the nutritional assessment guidelines and any physician dietary restriction noted on admission or incorporated later during their residence at this facility from a medically dietary adjustment by their physician

To routinely perform physical plant inspection daily by staff, monthly impromptu inspections by administrator to promote and maintain ongoing environmental health and safety awareness along with any infection control issues providing a safe and clean living environment at all times based on the best practice principles in this industry

To promote and provide ongoing training for all direct care staff and management staff based on the best practice principles in this industry
